PARTIAL HOSPITALIZATION PROGRAM

Overview
Our all-new Partial Hospitalization Program (
PHP) is an acute psychiatric program for adults and geriatric
population with a mental health disorder.  The PHP will target individuals that demonstrate a clinical need to
decrease a behavior or an exacerbating mental health condition that can result in a risk of being hospitalized or
a barrier to the functioning of daily life skills.  

The Goal of PHP

  • Stabilize the individual mental health condition, behavior or emotional state and transition to a less
    intensive service.
  • Provide patients with a support system

Services are provided through a patient center based multi disciplinary team approach under the direction of a
Psychiatrist.  The team includes a Psychiatrist, Licensed Nurse, LCSW and Social Workers.  Hope, Unity and
Freedom will utilize an individualized service plan to identify the patient’s
specific needs.  Goals and objectives
will be structured and related to the patient needs specific.  The plan will identify strategies for treatment to
stabilize the patient. Patients will be discharged from the program once the goals and objectives as assessed
are completed or the individual is no longer at risk of psychiatric hospitalization.  The discharge plan will
include recommendations for transition the patient to a less intensive day treatment.
